Concussion Information and Form

In 2013, the Tennessee General Assembly enacted Public Chapter 148. This act, effective January 1, 2014, requires school and community organizations sponsoring youth athletic activities establish guidelines to inform and educate coaches, youth athletes and other adults involved in youth athletics about the nature, risk, and symptoms of concussion and head injury.

As part of this act, each athlete must complete a concussion statement prior to participating in any youth sport.

Cardiac Arrest Information and Form

In 2015, the Tennessee General Assembly enacted Public Chapter 325. This act, effective July 1, 2015, requires school and community organizations sponsoring youth athletic activities establish guidelines to inform and educate coaches, youth athletes and other adults involved in youth athletics about the nature, risk, and symptoms of sudden cardiac arrest, including the risks associated with continuing to play or practice after experiencing symptoms.

As part of this act, each athlete must complete a Sudden Cardiac Arrest Information Sheet prior to participating in any youth sport.
